As you begin your health coaching journey with us, we begin a relationship together that will potentially take course over the following months and/or years to come. As inherent in our health coaching model, we will want to understand the complexity of who you are, your personal story, the triggers and precipitating factors that have impacted your health, and where this has brought you to date. This discovery process is part of us getting to know you, your history and your needs, and helps us formulate a treatment plan that is specifically catered to you. There are many facets to this initial discovery process.
You will be asked to complete and submit the following patient questionnaires prior to scheduling your appointment. We may recommend laboratory diagnostics to be performed prior to your coaching appointment so we can personally-tailor your coaching experience based on your specific needs and goals. Please do not submit screenshots of your medical records. We require copies of your original medical documents.

Exactly where you should be.
When you’re true to who you are, you can be true to where you are. Our coaching services are purposely designed for patients who take their health and their goals seriously. This coaching package can be a truly disruptive and transformative process for those willing to fully invest in themselves.
Make the Most of Your Coaching Experience
Do the Exercises. Answer all the self-assessments honestly, completely, and with vulnerability. This is not the place to defend our egos. Embrace your strengths and weaknesses, “areas to improve.” Progress can only be made if we’re willing to look inward with a clear lense.
Take time to reflect. The exercises are best done one, two, or, at most, three in one sitting. Do a few, and then come back to the concepts and next exercises later - this pace will give you some time to digest what you’ve learned so each session can build off previous ones. Going too quickly through the exercises often results in missing some of the most useful and transformative experiences.
Think and feel as you go. Progress requires reflection - this means thinking and noticing your feelings about the concepts and insights you develop from the exercises.
Pay attention to how, when, and where you use your coaching resources. Your state determines your story. Your story determines your strategy. If you are like most high achievers, you will probably try and squeeze these learning resources into your time on planes, at the end of the day, etc. We strongly recommend you give yourself the gift of focus when you are at your best. You want the focused, clear-headed version of you offering logical, rationally-based solutions - not the tired, distracted, version of you making emotionally-guided solutions.
Don’t rush it. It took you years (or decades) to get to this point. You will not find a desired path to the future in a few minutes (or even a few hours). Give yourself the gift of time and realistic expectations.
Consider working with a friend, partner, or loved one (someone who knows the real you and won't shy away from honest feedback). Often, people find talking with trusted confidants helps them extract additional insight from the thoughts and feelings evoked by the exercises.
Clarity doesn’t strike, it unfolds. At times we offer educational resources that might overlap in helping you craft your vision of a desired future and your plan to get there. If you have achieved tremendous insight from a few exercises, and are reading another that seems familiar or redundant, you may spend less time and energy on it. But remember, each exercise is has a distinct purpose and meaning, tied carefully to those that come before and after. It is best to try and do them all, and in order.
